Why this template exists

In a serious emergency, you may not be able to speak. The paramedic treating you doesn’t know your blood type, your allergies, or that you’re on a medication that interacts with a common treatment.

This card takes five minutes to fill in and can change the outcome of an emergency. Most people assume their phone’s medical ID is enough — but a locked phone at an accident scene is no help to anyone. A card in your wallet is.

Once you’ve filled it in, keep a scan of it alongside your other important health documents. filedup is built for exactly that — private, on-device document storage for the things that matter.
